IMPORTANT: New requirement for working on-site at MMC - Attestations

The Victorian Government now requires that all staff and students entering healthcare services make a daily attestation about their health status with respect to COVID-19.

This is aimed at making workplaces safer for healthcare workers, and ensuring staff do not come on-site if they have symptoms or close contact with infected persons.

Before coming on-site, you will need to submit a form online using a smartphone, and show the result to our screening team when you receive a thermal scan. You can also fill in a printed form and submit it on entry (forms will be available at entrances). This takes less than a minute to complete. You can also save a shortcut to the form on a smartphone to make the process faster.

Note: for "Employee Number" please enter "Uni".

The locations for entering MMC under current restrictions are attached to this email again, for reference.

This requirement applies to all staff who work on-site at the Monash Medical Centre precinct, including MIMR, MHRP, TRF and MCH buildings. It does not apply to the BASE Facility.
