Adolescents who are overweight or obese lose weight on commercially delivered weight management program

Monash University research has shown that a commercial,
structured weight management program leads to weight loss and improved
psychosocial outcomes in adolescents who are overweight and obese.
Obesity in childhood and adolescence tracks into adulthood,
and is associated with adverse physiological and psychological health outcomes.
“In Australia there are limited effective services and tools
for physicians to manage adolescent obesity—and few children and adolescents
with obesity seek treatment,” said lead researcher Associate Professor Maxine
Bonham from the Department of Nutrition, Dietetics and Food at Monash
University.
Associate Professor Bonham said results from a 2011 Cochrane
Review had suggested more widely accessible interventions for adolescents with
modest weight issues prior to the development of complications would be
advantageous.
Published last week in The Journal of Pediatrics, the
Monash University research shows for the first time that a structured
life-style intervention delivered by a commercial provider in an adolescent
population results in clinically relevant weight loss.
“Commercial diet
providers are a popular option for adults wanting to lose weight, and if food
is supplied as part of the program, reported weight loss is greater,” said Associate
Professor Bonham.
For children and adolescents
with obesity, however, commercial providers of weight management are scarce.

Bonham’s study evaluated an adolescent weight management program, JenMe, a
12-week face-to-face program developed by dietitians at commercial provider
Jenny Craig.
From March
2013 to March 2014, 88 adolescents (aged between 13 and 17) who were overweight
and obese participated in the study, were allocated to either the intervention
group or the wait-list control group.
“A
significant weight reduction of 6.1 kg was observed in the 32 participants who
completed the intervention, compared with an average weight gain of 1.9 kg in
the 22 participants who completed the no-treatment period,” said Associate Professor
Bonham.
Professor
Bonham said the intervention participants also reported greater body esteem and
quality of life compared with the wait-list control group.
Although
follow up data were only available for 35% of intervention group participants,
on average they had maintained their weight loss in the 24 weeks after
completing the JenMe program.
Commercial weight loss operators provide extended reach and
resources beyond that which can be realistically delivered by an already
stretched health service.
“Some preliminary data also indicate the affordability and
viability of commercial diet programs compared with weight loss interventions
run by health professionals,” said Professor Bonham.
“Delivery of such programs by commercial providers offers an
alternative avenue to standard care in the management of paediatric obesity.”

Shift workers’ health to benefit from heart disease grant

A leading Monash University nutrition scientist has
received the 2016 National Heart Foundation Ross Hohnen Award for Research
Excellence for her project to improve heart health in shift workers.
The award recognises Associate Professor Maxine Bonham’s outstanding and
innovative Vanguard Grant application, with the award providing $10,000 on top
of the $65,000 grant.
Associate Professor Bonham’s research aims to improve shift workers’
heart health through development of a
dietary strategy that modifies the timing and amount of food consumed at night.
“Shift work is a risk factor for cardiovascular disease (CVD)
probably as a result of the disruption to normal circadian rhythms experienced
by people who are awake overnight and sleep during the day," said Associate
Professor Bonham.
“Eating throughout the night
contributes significantly to this risk by forcing the body to actively process
food at night when usually we are asleep.”
“The same meal consumed at night
rather than during the day results in higher circulating blood lipids and
glucose which are both independent risk factors for CVD. This becomes more pertinent when we consider
that shift workers eat a substantial proportion of their daily energy intake
during this overnight period.”
Associate Professor Bonham’s study
aims to show that modifying the timing and amount of food consumed at night in
a shift working population will impact favourably on blood lipids and glucose
in the short term with long term benefits in obesity prevention.
“Current dietary recommendations may not be directly applicable to
shift workers owing to the impact of circadian misalignment on physiological
and metabolic processes.”
“Our research will produce evidence to
inform guidelines promoting cardiovascular health for shift workers,” said
Associate Professor Bonham.
Associate
Professor Bonham said she is honoured
to have been awarded both a Vanguard grant and the Ross Hohnen award for
research excellence and acknowledges her colleagues and collaborators Dr
Catherine Huggins, Dr Sletten, Dr Murgia and Associate Professor Young.
Monash Health Translation Precinct (MHTP)
researchers Dr Nadine Andrew and Associate Professor Lisa Moran also received
competitive Heart Foundation research grants this round.
Dr Andrew from the Stroke and Ageing
Research Group was awarded a Vanguard Grant for her project to develop
standardized methods for setting patient centred goals in stroke while
Associate Professor Moran’s (Monash Centre for Health Research and
Implementation) Future Leader Fellowship helps enable her research into
optimizing weight management in women of reproductive age.

Congratulations to nutrition scientist Associate Professor Maxine Bonham whose work in research and
clinical education was recognised in the latest round of academic promotions.
A
registered nutritionist, Associate Professor Bonham’s early research career at
the University of Ulster focused on the risk-benefits of fish consumption
during pregnancy on neurocognitive outcome; collaborative work that has
resulted in a change to World Health Organisation advice regarding fish
consumption during pregnancy.
Since
starting at Monash as a Senior Lecturer in 2010, Associate Professor Bonham has
successfully established an undergraduate program in Nutrition Science and in
2015 the course was granted specialist course status under the new Monash
Course Architecture.
Associate
Professor Bonham’s current role in the Department of Nutrition and Dietetics is
spilt between a commitment to education and pursuing her research interests.
“I
always thought I would remain in a 100% research role but the opportunity to
work with undergraduate students, see them get excited about research, and
progress into PhD and post-doctoral careers is extremely rewarding,” said
Associate Professor Bonham.
Associate
Professor Bonham’s research interests are focused on the role diet plays in
metabolic health outcomes. Recent projects include the evaluation of a
commercial weight loss program in overweight adolescents and investigating the
effect dietary manipulation can have on appetite. Current research
examining the effect of meal timing on glucose and lipid metabolism has
implications for people in occupations that require them to eat late into the
night.
“We
have shown that a meal consumed late at night compared to the morning greatly
impacts on glucose metabolism. This may have implications for the 16% of
the Australian population in shift work roles.”
